Description

The purpose of this project is to support the National Council on Aging’s Center for Healthy Aging (CHA). CHA houses two resource centers that are funded by the U.S. Administration for Community Living (ACL), Administration on Aging: The National Falls Prevention Resource Center and the National Chronic Disease Self-Management Education Center. These Centers work collaboratively on behalf of the public, aging services network, and other stakeholders to accomplish the following goals:

  • Provide technical assistance, education, and resources to increase public awareness about the risk of falls and how to prevent them.
  • Support and stimulate integrated, sustainable service networks within states/regions/tribal entities that provide falls prevention and evidence-based health promotion and disease prevention programs.
  • Support the expansion and sustainability of falls prevention and evidence-based health promotion and disease prevention programs.

Since 2014, NCOA’s Center for Healthy Aging has supported over 75 ACL state, local, and tribal grantees. These grantees collectively have had over 220,000 older adult participants in evidence-based health promotion and disease prevention programs and more than 80,000 participants in falls prevention programs.
