Description
The DSMES Toolkit for public health and clinical providers aims to assure that people with diabetes have access to evidence-based education and support programs to help manage their condition. Developed in collaboration with CDC, the electronic DSMES Toolkit provides easy access to information and tools needed by health care professionals to provide DSMES services. Additionally, 6 national webinars, marketed to 1305 state grantees to increase the utilization of DSMES programs among patients and health care professionals were conducted. The 1305 state grantees encompass 4 CDC programs (diabetes, heart disease and stroke prevention, nutrition, physical activity, and obesity and school health) working together to support chronic disease prevention programs nationwide.
