Developed in collaboration with CDC, the electronic DSMES Toolkit provides easy access to information and tools needed by health care professionals to provide DSMES services. Additionally, 6 national webinars, marketed to 1305 state grantees to increase the utilization of DSMES programs among patients and health care professionals were conducted. The 1305 state grantees encompass 4 CDC programs (diabetes, heart disease and stroke prevention, nutrition, physical activity, and obesity and school health) working together to support chronic disease prevention programs nationwide.
